Education and Experience: High school diploma or equivalent. Undergraduate degree in social work or behavioral science preferred. One to two years of non-profit or related experience strongly preferred. Experience working with the homeless population preferred.
Licenses and Certifications: Valid Oklahoma Driver’s license, satisfactory driving record, and proof of automobile insurance coverage.
Knowledge: Oral and written communication ability equivalent to that normally acquired through a high school education. Basic knowledge of service provision to homeless community. Knowledge of current Federal, State and local laws, regulatory codes, ordinances, procedures relevant to homeless assistance programs, such as those designed to improve/provide housing, education, employment, or socio-economic status. Knowledge of community resources available in Norman, Cleveland County and Oklahoma City.
Skills: Working closely with a multidisciplinary team and assisting Continuum of Care partnering agencies; maintaining a professional, customer service-oriented attitude in a diverse, collaborative team environment; strong advocacy skills. Setting and observing appropriate boundaries with clients; basic understanding of severe mental health, physical health, and substance abuse issues and symptoms. Well-developed decision making skills, attention to detail with a high level of accuracy. Multi-tasking and remaining calm in demanding and unpredictable situations; using tact, discretion, initiative and independent judgment within established guidelines. Meeting face-to-face with homeless individuals of diverse backgrounds and disabilities, including physical, mental, substance abuse, HIV/AIDS, etc.; developing rapport, providing information, and referring with sensitivity to cultural issues. Excellent time management skills and the ability to work with minimum supervision. Proficient use of a computer and laptop; Microsoft Word, Excel and Outlook.
Duties and Responsibilities: Provides field based/mobile case management services using evidence-based practices to persons experiencing long term homelessness. Pursues an engagement process seeking to build a relationship of trust and support that involves outreach, screening, intake, assessment, individual service plan development, monitoring, linkage to appropriate community resources, follow-up, advocacy, appropriate discharge, and tracking tenant outcomes. Supports individuals experiencing homelessness through the following activities: one-on-one contact with individuals on the streets and in emergency shelters; advocating on behalf of clients and/or supporting them to advocate for themselves; collaboration and coordination of services with appropriate community agencies. Provides updates at weekly Coordinated Case Management and all Continuum of Care related meetings. Maintains client related data tracking systems, including case notes and complete HMIS entries; prepares case-related reports; generates client data for monthly reporting. Works collaboratively with the Outreach Housing Liaison and other service providers to conduct outreach in the community, focusing on those who are most vulnerable. Assists in facilitating monthly Homeless Outreach Task Force meetings. Gains knowledge of the contracts and grants which Continuum of Care partners work under; monitors proposed changes to programs and associated software. Assists in obtaining housing readiness documentation. Maintains complete mileage logs, daily activity logs, and other reports as directed. Develops and maintains excellent working relationships with community partners, coalitions, and resources; works with collaborative teams to find solutions for individual clients as well as system-level issues; advocates for needed services and individual clients. Completes Vulnerability Index Service Prioritization and Decision Assistance Tool (VI-SPDAT) to determine acuity during street outreach, as well as other outreach and referral processes. Assists people experiencing homelessness with accessing resources and making referrals; continues to engage and develop trust with unsheltered neighbors until shelter or placement in a housing program is obtained. Responds to referrals and client’s request for case management assessment and intervention; responds promptly to all calls, emails, and communication requesting outreach for a certain individual or area. Serves as a point of contact for business districts, business owners, community members, etc. who have concerns about specific individual or camps; provides information and tools, and conducts sensitivity training with the Outreach Housing Liaison. Performs other related duties as required.
Working Conditions: Work is performed in an office and public, service-delivery environment. Hectic situations can occur in the public area of the facility characteristic to working with the homeless community and the concerns and situations related to homeless conditions. Potential exists for volatile and unpredictable situations to occur. May experience exposure to strong odors such as soiled clothes and poor hygiene. Must be comfortable with flex scheduling and willing to respond to emergency outreach situations after-hours if necessary.
Mental and Physical Abilities: Ability to maintain regular, predictable and punctual attendance. Ability to interpret and follow oral and written instructions. Ability to concentrate and pay close attention to detail with constant interruptions associated with answering phones or completing the intake process in the field and in shelter environments. Moderate mobility; field duties may be performed by frequent walking and/or standing, kneeling, or squatting relieved by lesser periods of sitting. Ability to work outdoors frequently, particularly in extreme weather.
